Specialist integrated health and social care services are provided by 3 Community Learning Disability Teams (CLDTs) for people with a learning disability (PWLD) who live in Forth Valley. There is also the 20-bed in-patient unit at Lochview hospital.
Clackmannanshire and Stirling who are jointly managed within the Clackmannanshire and Stirling Health and Social Care Partnership, and Falkirk who are managed within Falkirk Health and Social Care Partnership (HSCPs).
Lochview provides care for people with a learning disabilities who require specalist or complex health assessment and treatment which cannot, at that time be met in the community either by generic health services or the CLDTs. Within Lochview a multi agency, therapeutic model of care is delivered which promotes rehabilitation and social inclusion. Staffs from Lochview work closely with CLDT staff members.
The CLDTs consist of health and social work staff providing specialist assessment and intervention work for PWLD who have complex health and social care needs. CLDTs work with other health and social care services to meet the needs of each individual.
