If you are concerned about your memory you should contact your GP. The GP will eliminate potential causes in the first instance then complete routine dementia cognitive screening, which may include bloods and memory tests. The GP will refer on to the appropriate service for older adults for whom dementia is suspected for further investigation and diagnosis.
For individuals who have a diagnosis of Learning Disability, any concerns about changes in cognition should be directed to the Learning Disability Memory Multi-Disciplinary Team (MMDT). The MMDT will liaise with the service user and his/her support network to offer a thorough dementia screening and assessment, and post diagnostic support for those who receive the diagnosis of dementia.
