Getting to Know Me | Alzheimer Scotland – This is a simple tool that can be offered to anyone but is particularly useful for those with dementia. The document allows us to get to know the person, understand who and what is important to them and what their likes and dislikes are. This promotes person centred care and ideally should be offered at the earliest opportunity.
