What we do
Keep Well is a community based service which provides a range of health assessments, along with tailored advice and support, to help improve health and wellbeing and prevent disease.
The service provides one-to-one health assessments for both men and women, over the age of 18, which cover a wide range of health issues including:
- Checking cholesterol
- Taking blood pressure
- Measuring weight
- Assessing diabetes risk and taking blood samples, if required
- Healthy heart advice
- Mental wellbeing
- Men’s health
- Women’s health
- Healthy living
There is also an opportunity to talk about anything else that may be worrying you about your health.
After the health assessment, staff will work with you to improve or maintain your health. For example, staff can support you to:-
- Eat healthier
- Lose weight
- Increase your physical activity
- Stop smoking with personalised support
- Cope with stress
- Respond to any work related health issues
Staff can also put you in touch with other local services, if needed.
The Justice Keep Well nurse also accepts referrals for people who present in the Justice Service in order to assess their physical, mental and emotional health needs. Health-related advice and support is also offered to case managers to assist them in their supporting role within the service.
