The Stroke Liaison Service provides:
- Education, care, and support for up to one year after a stroke diagnosis.
- Support and advice for family and carers.
- Discussion of issues important to you, such as diet, activity levels, and medication.
- Help with any problems affecting your independence and quality of life.
If you are a patient with a confirmed stroke, the Stroke Liaison Nurse will contact you by telephone about two weeks after discharge for a follow-up. You will also be invited to a further follow-up after six months, either in person or by phone.
If you have had a TIA (Transient Ischaemic Attack), routine follow-up is not provided, but you are welcome to contact the service for advice.
