When you have T2DM there are several differing services that may support you. Primarily your GP will be the service helping to coordinate your care and help you to decide which is the best medication and lifestyle combination to help you reach your health goals.
You will be asked to attend a yearly screening appointment with either your GP, local CTAC service or at the specialist clinics at FVRH or SCH. This appointment gathers all the information to then give you an opportunity to determine what type of support you require to manage your diabetes.
You will have bloods taken, these include your HbA1c and U&E’s, blood pressure checked a urine sample for ACR, foot check, glucose monitoring device downloaded, and a lifestyle questionnaire.
The person at this appointment will be a health care support worker who has been trained to complete the above process, but they will not be able to discuss your health or what the results mean.
After your screening appointment you will receive a letter with your screening results. This will explain what they mean and will then give you the opportunity to make an appointment with the health care professional who supports you with your diabetes management.
