Patient opinion is very important in the forward planning of care and services and as such we are very keen to gain feedback and learn from your experiences. This questionnaire was designed for use by women and their partners who have experienced the loss of their baby/babies during pregnancy or shortly after birth.

Please accept our condolences on the loss of your baby and our gratitude for the time taken to complete the questionnaire which we understand can be difficult. Please complete as much or as little of the form that you are able to. Some questions may not be relevant to your experience. Please tick the relevant box for each question and write any comments you may have.

Give consideration to any aspects of the care you received before, during and after delivery of your baby commenting on what mattered to you and your partner/family, what was important to you, what impacted on your experience and what could have been done differently to improve on the care you received. All information will remain anonymous unless you wish to provide your contact details at the end of this questionnaire.

If you would like to discuss any aspect of your care, please contact your team midwife or a member of the bereavement team at the hospital.

    Parent Feedback Questionnaire

    1. I was cared for in an appropriate environment during and following the delivery of my baby/babies.

    2. NHS staff communicated with me/us in a sensitive way.

    3. I felt confident in the NHS staff caring for us.

    4. I was able to be involved in caring for and making any decisions about my baby/babies.

    5. I was fully informed about what had happened to my baby/babies.

    6. I was given the opportunity to spend the time I wanted in the way I wanted with my baby/babies.

    7. I was given the opportunity to create memories with my baby/babies in the time I needed without feeling rushed (eg photography, lock of hair, foot and handprints, washing and dressing, taking my baby/babies home etc).

    8. I felt that my baby/babies were treated with dignity, respect and sensitivity at all times.

    9. My family members and my other children were included appropriately in my care.

    10a. Adequate explanation, guidance and support was given when asked about the next steps for my baby/babies (eg such as
    whether to have a postmortem or not).

    10b. I was given enough time to make my decisions.

    11a. I was given enough information about burial, cremation and the costs if any, associated with them.

    11b. I was given enough time to decide and make clear what I wanted to arrange/have arranged for my baby/babies.

    12. I felt assured that my GP and community midwife had been informed of my loss before I left the hospital.

    13a. I was given the bereavement team information leaflet with details of how to contact them.

    13b. I received a follow up contact card from the bereavement team after leaving the hospital.

    13c. I found this information to be helpful.

    14. I was given details of relevant support groups, counselling services, organisations and websites.
