In May 2022, the Scottish Government committed £2 million every year to NHS Boards to implement annual health checks for people with learning disabilities across Scotland.

Annual health checks have been shown to be effective in identifying previously unmet health needs and improving the management of long-term conditions.

In Forth Valley, the Keep Well Service offers a free annual health assessment for people with learning disabilities who are registered with a GP.

Invitations to book an appointment are sent throughout the year.

Why have a health check?

Your annual health check helps make sure you stay healthy and well.

The check looks at both your physical and mental health and provides an opportunity to discuss any concerns you may have.

Before your appointment

Shortly before your appointment, you will be sent a questionnaire to complete.

• A carer, family member or someone important to you can help you complete it.
• If you find the questionnaire difficult to complete, you can still attend your health check.

What happens at the appointment?

• You will be seen by a nurse from the Keep Well Service.
• The appointment can take up to one hour.
• You can ask to stop the appointment at any time.
• A supporter, carer or trusted person can attend with you.

After your appointment

Following your health check, we will write to your GP to update your health records.

If appropriate, we may also refer you to another service and share relevant information with them.
