Depending on the device you are using, we will ask you to upload to different platforms.
We want to help you to be able to do this from home. This will help you to review your own data between clinic visits and become more confident with looking after your/your child’s diabetes. It also means that you do not have to come into hospital simply to share data with us as we can see it remotely.
Here are the common systems we use for upload:
Diasend
The following devices can be uploaded to Diasend:
- AccuCheck glucose meter
- Omnipod and Tandem (T-slim) insulin pumps
- Dexcom continuous glucose monitoring system
Diasend have helpful advice on how to set up an account and upload on their website
We will email you a link to connect to our clinic so that we can see the data jointly.
Medtronic Carelink (Personal)
If you use a Medtronic pump, you can upload it to CareLink Personal. Before you upload for the first time you should click on “How to Install Uploader” and follow the instructions.

Dexcom Clarity
The application used by Dexcom CGM is called Clarity. To allow sharing of data, let us know to send an invitation by e-mail.
If you use the Clarity app, you can also share by clicking on “Profile” at the bottom of the page and follow the instructions to “Authorise Sharing.”
In order to share your CareLink Personal data with the diabetes team we need you to provide us with your username and password so that we can set up a link to our “clinic” version of CareLink.
LibreView
LibreView is how you can review and examine your numbers from your Libre Sensor at home. Once linked to our log in, we can see this information as well.
Visit the LibreView website to upload results from the FreeStyle Libre. If you use the mobile app, data will automatically be uploaded to it but you need to connect the scanner that comes with the starter pack to a PC in order to upload the data from it.
In either case, to share data with the team go to the Settings section (the three lines in the top right corner of the webpage) and click on Account Settings. Now click on My Practices and enter this number in the box that says Practice ID: 10148103.
You should now see our clinic name appear on the list. Alternatively you can ask us to send you an e-mail invitation to share the data.
MyLife
The myLife app is a mobile phone app used as an insulin bolus dose calculator. You can set up your individualised time blocks with your Insulin to Carbohydrate Ratio (ICR), Insulin Sensitivity Factor (ISF) and target glucose range.
You insert a blood glucose or scan glucose reading, your carbohydrates and it then advises a bolus dose.
The app can give you information about your insulin doses, carbohydrates consumed and blood glucose levels which it can also summarise into a PDF.
The app unfortunately does not currently have the ability for sick day or exercise function.
