The Allied Health Professions (AHP) Rehab Care Group provides coordinated assessment and rehabilitation services for adults across Forth Valley.
Our AHP team includes Physiotherapists, Occupational Therapists, Speech and Language Therapists, Dietitians, and Therapy Assistant Practitioners. We provide a range of community services, including support in community hospitals and the Stirling Health & Care Village.
Wherever possible, services are delivered locally, with access to Forth Valley wide specialist teams for patients with complex needs.
The Care Group includes
- ReACH North – covering Stirling, Rural, and Clackmannanshire localities
- ReACH Falkirk – covering all Falkirk localities
- Neuro Rehabilitation Team
- Community Hospital Teams
- Spasticity Management Service
- Adult Speech and Language Therapy
Teams work in a variety of settings, including people’s homes, clinics, care homes, and inpatient facilities where appropriate.
A Single Point of Referral (SPR) is available for anyone needing assistance from the ReACH teams or for spasticity management.
Teams work collaboratively with service users, families, carers, and Social Work to support discharge planning and self-management at the earliest opportunity. A key part of therapy involves working towards patient-centred goals, which often include:
- Supporting individuals to manage their own conditions and remain well at home
- Managing safety
- Maximising functional independence
- Restoring function and quality of life to optimum levels
- Preventing unnecessary hospital admissions and enabling safe, early discharge
