What is Patient Hub?
Patient Hub is an online portal designed to provide quick and easy access to essential information about your care. You’ll be able to securely access it from your smartphone, tablet, or computer.
Once fully launched, Patient Hub will provide:
- A waiting well service keeping you updated on your referral to us
- Details of your hospital appointments
- Health questionnaires and surveys relating to your appointment
- Key information related to your condition and care
Introducing Patient Hub
Patient Hub will be introduced at the end of January 2025 with the following initial features:
- Notifications sent to your mobile phone, email address, or landline to inform you when a referral has been accepted onto a waiting list.
- Additional notifications with a link to Patient Hub, where you can access information about your referral. This may include supporting information leaflets and patient tasks to complete before your first appointment.
Later this year attendees of some Outpatient Clinics will begin to access appointment information via Patient Hub, an online portal allowing them to confirm, reschedule, or cancel appointments online using a smartphone, tablet, or computer.
Patients with a registered mobile number or email will receive automatic access. Please provide your current contact details during your next hospital or GP visit. Once clinics are online, you’ll receive a text or email with a link to view your appointment letter instead of receiving it by post. If the link remains unopened after three days, a printed letter will be mailed to you.
This online access is convenient for syncing with digital calendars and helps the NHS save on postage and reduce paper use.

- You’ll be asked to enter your mobile number or email address and your date of birth.
- You will then be sent a 6-digit authentication code by text message or email.
- Enter the 6-digit code. Please be aware the authentication code will expire in two minutes and 30 seconds.
- Then you are securely logged into Patient Hub.
Referrals to Outpatient Services
- From January 2025 patients will start receiving notifications from this date to advise them their new referral has been accepted and added to a Waiting List.
- We are working towards expanding this messaging service for new referrals, providing support and information to patients ahead of their appointment. Information and timescales will be published here. Any further information will ask you to log onto the Patient Hub.
Waiting List Validation
- Later this year, the Patient Hub will be used to confirm whether your referral or appointment is still needed while you remain on the waiting list.
Appointment Information
- In the future this you will be able to manage appointments and view digital copies of letters and service correspondence in Patient Hub.
If you request a rebook or cancel your appointment, someone from the appointment booking office will contact you within two working days.
